In order to further complete the preparation of the "Guidelines for Diagnosis and Treatment of Psoriasis in China" (2022 Edition), the first mid-term research forum of the 2022 edition of the guideline was successfully held online from May 3 to 4, 2022. The editor-in-chief Professor Zhang Xuejun and the consultant Professor Zheng Jie , Prof. Zheng Min, Prof. Yang Zhibo, associate editors Prof. Wang Gang, Prof. Zhang Furen, Prof. Cui Yong, Prof. Li Bin, Prof. Zhang Xibao (and secretary-general), editorial board and secretary, a total of 31 people attended the conference. The forum was co-sponsored by the editorial board of "Guidelines for Diagnosis and Treatment of Psoriasis in China" (2022 Edition) and the Psoriasis Professional Committee of the Dermatology and Venereology Branch of the Chinese Medical Association, undertaken by the China Medical Equipment Association, and supported by Beijing Novartis Pharmaceuticals Co., Ltd.

Professor Zhang Xuejun, the editor-in-chief, delivered a speech first, expressing his affirmation and gratitude to each editorial committee for their work in compiling the guidelines for overtime seminars during Labor Day, and introduced that the mid-term research forum is divided into five special sessions, including those on topical and traditional medicines, and biological preparations. And small molecule targeted drug sessions, treatment plans and other treatments sessions, traditional Chinese medicine sessions and diagnosis, auxiliary diagnosis and typing sessions. It is pointed out that in order to improve efficiency, the forum adopts the form of each editorial committee report and raises questions that need to be discussed in this part of the content. It is hoped that the editorial committee can combine the latest international progress and guidelines, keep pace with the times, and further update on the basis of the 2018 version. At the same time, the content that has been affirmed in the field of psoriasis in my country is written into the guideline. It is hoped that every editorial board member will actively participate and complete the guideline seminar with high quality.

At the meeting, the editorial board members of each special session actively spoke on the basis of the problems, conducted rigorous exchanges and discussions on the content of the guide, and put forward reasonable suggestions for revision. The topic of topical medicines and traditional medicines was chaired and summarized by Professor Zhang Xibao. This session discussed the types and concentrations of topical medicines, the writing of new foreign medicines, and the use of topical medicines and bathing therapy; traditional medicines such as methotrexate The status and dosage recommendations of psoriasis, cyclosporine, tretinoin, glucocorticoids, etc. in the current treatment of psoriasis.

Biologics and Small Molecule Targeted Drugs Session was chaired by Prof. Wang Gang, and Prof. Zhang Xuejun concluded that the session identified the methodological issues of guideline writing. The guideline writing needs to be registered in advance, and the selection of references should be high (quality) cited, academic It is widely recognized in the world, and a reference selection group needs to be established. The pathogenesis of psoriasis includes my country's progress in genetics, immunity, environment, etc., as well as the writing of new international and domestic organizations; Small molecule targeted drugs, sorting of different types of biological preparations, table display format, writing of unmarketed drugs, etc.

The treatment plan and other treatment sessions were presided over by Professor Zhang Furen and summarized by Professor Zheng Jie. During the special discussion, it was pointed out that the chemical name of the drug name was used instead of the trade name, the scope of special types of arthropathy psoriasis, and the symptoms of guttate psoriasis. Treatment, the psychological impact of psoriasis on children and adolescents should be more of a concern, as well as physical therapy for psoriasis, choice of treatment options, integration, treatment of special populations and duplications in between.

Treatment options and other treatment sessions

The traditional Chinese medicine session was chaired by Professor Li Bin and concluded by Professor Yang Zhibo. The special session discussed the etiology and pathogenesis, various symptoms and clinical characteristics of psoriasis from the perspective of traditional Chinese medicine, the basic principles and recommended prescriptions of traditional Chinese medicine, external treatment of traditional Chinese medicine, and non-traditional Chinese medicine. Drug therapy, oral Chinese patent medicine, management and prevention of chronic diseases in traditional Chinese medicine for psoriasis; determine the requirements of some references in traditional Chinese medicine, the interaction between traditional Chinese medicine, Chinese patent medicine and biological preparations, the basic syndrome types, comorbidity treatment, new additions The problem of prescriptions, the choice of drugs for external treatment, the order of writing, the length of other treatment plans, etc.

Chinese medicine session

The special session on diagnosis, auxiliary diagnosis and classification was chaired by Professor Cui Yong and concluded by Professor Zheng Min. The special session discussed the differential diagnosis, auxiliary diagnosis, classification, severity classification, comorbidities of psoriasis, and chronic psoriasis. For management and prevention problems, the diagnosis of imaging should focus on the joint disease type. The classification of psoriasis is recommended to be written separately from the drip type and the plaque type. It is recommended to continue the 2018 version of the three-point method for severity classification. Rigorous, convenient and suitable for use by outpatient doctors, BSA is currently more practical in clinical practice. At the same time, the DLQI of psoriasis and the international treatment concept for early and rapid resolution of patients' conditions and reducing comorbidities in psoriasis treatment should be considered; The inclusion of comorbidities in the narrow sense is the highest, and the inclusion of tumors and psoriatic arthritis is not recommended.

Diagnosis, auxiliary diagnosis and typing session

At the end of the meeting, Secretary-General Professor Zhang Xibao first concluded, pointing out that after two days of enriching and academic discussions, from the writing mode of targeted treatment of psoriasis, the content and attention should be increased in psychotherapy, classification methods, severity classification, traditional Chinese medicine Some of the current first drafts have a lot of content, and should basically reach a consensus on the length and other aspects. The 2022 version of the guide should update 20%-30 of the content on the basis of the 2018 version. I believe that on the basis of writing four years ago, each editorial board will be more. Calm and determined to compile a new version of the guide. The secretariat team will also fully and actively cooperate with each editorial committee to escort the compilation of the guide. Editor-in-chief Zhang Xuejun then made a concluding speech for the entire mid-term seminar, pointing out that our guideline is not only used by clinicians, but also read by psoriasis patients. The process of compiling the guideline is also a process of learning knowledge, imparting knowledge, applying knowledge, and creating knowledge. and the process of summarizing knowledge. The editorial board of the 2022 guideline is my country's authoritative experts in the field of psoriasis, including the old, middle and young generations. It is pointed out that the content of this version of the guideline should be consistent with the 2018 version. It is suggested that the biologics section can be appropriately concentrated, and it is better to express it in the form of diagrams. At the same time, it is combined with the goals of Chinese medical insurance and European and American expert groups and guidelines for psoriasis treatment. and recommendations; emphasised that the comorbidity and psychotherapy components should be increased. Once again, it is pointed out that excluding TCM references, the rest of the references should be mainly SCI papers, which should be cited by high-quality foreign articles, classic textbooks, guides, etc., and a reference selection group should be established. After two days of meetings, more than 90% of the issues reached consensus during the meeting, and the rest of the issues could continue to be exchanged after the meeting. Editor-in-chief Zhang Xuejun finally pointed out that the revised draft of the guidelines after the meeting will be handed over at the end of May, and the second mid-term research forum is tentatively scheduled to be held on June 18. It is hoped that all editorial members will work together to contribute to the development of the prevention, diagnosis and treatment of psoriasis in my country.
